 The Position:

CTN will coordinate the care transition of the patient to include gathering chart information, collaborates with referring and/or attending physician to ensure hospice eligibility and establish the Plan of Care, Provides daily GIP visit services or respite care when patient resides in the hospital, provide education and in-services to hospital personnel as well as work with the sales team to establish and maintain positive working relationships with current and potential referral sources, may also provide case management of patients outside of the hospital and attend Interdisciplinary Team Meetings.

The Requirements:

  • Registered nurse with current licensure with no restrictions to practice professional nursing in the state.
  • One (1) year experience as an RN in healthcare; with at least one of which is in public health, home care, or hospice experience preferred
  • Graduate of an accredited school of nursing with a BSN degree is perferred
  • Previous experience in both the hospice and hospital setting preferred.
  • Proven ability to work within an interdisciplinary setting.
