As the Director of Patient Care/Patient Care Manager for Heart of Hospice, you will oversee all clinical services as well as coordinate and manage patient care for your agency. The Director of Patient Care/Patient Care Manager helps Heart of Hospice fulfill its mission and vision by ensuring all patients who choose our services are given personalized care plans to meet their unique needs. This position works closely with RN Case Managers and communicates effectively with all interdisciplinary team members to ensure quality service and compliance with patient care plans.

The Position:

The Director of Patient Care/Patient Care Manager is responsible for the overall direction of patient care by planning, coordinating, and directing hands-on, skilled nursing care provided by RN Case Managers and the interdisciplinary team, ensuring care plans meet patients’ goals of care. The Director of Patient Care/Patient Care Manager also oversees the logistical operations of the agency by assisting in recruiting and hiring staff, assuring maintenance of clinical records, and attaining the financial goals of the organization.


The Requirements:

  • Registered Nurse with current, unrestricted licensure in the state
  • Minimum of two years of patient care experience
  • Previous experience in hospice and/or home care settings with at least three years of management or supervisory experience
  • Current CPR certification
  • Hospice or palliative care certification preferred
