Bereavement Coordinator

Bereavement Coordinator is responsible to plan, implement, and maintain a bereavement program to help the family cope with death related grief and loss issues for up to 13 months following the death of the individual hospice patient as well as offer Bereavement to facilities where hospice patients reside.

As a member of the Bereavement Team, you will serve as a critical liaison between the benefit of hospice services and potential patients, their loved ones and caregivers, and our healthcare partners. The Position:

The Bereavement Coordinator will coordinate and supervise all aspects of the grieving process. He/she will also supervise the documentation and assessment in addition to providing grief counseling. This includes one-to-one, groups, and ongoing follow-up to ensure timely and appropriate services are provided to family/caregivers. Assists as needed in evaluating and planning services with hospice personnel, i.e., yearly retreats, weekly support groups, hospice activities such as in-service education, hospice personnel meetings, and relevant committees (Interdisciplinary team meetings) Coordinates the planning of memorial services along with the chaplain.

The Requirements:

  • Minimum 4-year degree; preferred (Social Worker or Licensed counselor)
  • Documented evidence of appropriate training and experience counseling and addressing grief issues of the bereaved under the supervision of a qualified professional.
  • Hospice/ experience preferred
