The Position:

The Palliative Care Coordinator (PCC) is responsible for coordinating all the palliative care activities and oversees the daily operation of palliative care program which will include preparing for all meeting and schedule meetings, coordinating all telephone calls, internal communications, creating and implementing provider schedules, tracking and reporting systems, workload of providers, maintain employee personnel record as well as identifying and resolving issues that arise for the providers in the field. Relationship management, medical records, maintaining patient records/data entry. The PCC will serve as the primary point-of-contact for customers. He/she will establish and maintaining contacts with each of our partners, according to the providers schedule, developing market plans and serving as a billing liaison. This person will need to have basic financial and business knowledge. The PCC will report to the Admissions Director.


 The Requirements:

  • Minimum of three (3) years of applicable clinical experience as a Social Worker, LPN or Minimum of five (3) years of direct administrative experience.
  • Physician Practice experience, particularly in management
  • Current state nursing license and in good standing with medical/nursing board, if applicable.
  • Minimum of three (3) years of experience working as part of a hospice/palliative care interdisciplinary team preferred
  • Strong public speaking skills required.
