The Chaplain is responsible for the provision of spiritual care services to patients and families/caregivers of the hospice, either directly or through coordination of care with other spiritual counselors. The Position:

The Chaplain will complete a spiritual assessment of patients and families/caregivers as well as keep documentation of the progress of the patient/family on the clinical progress note. He/she will coordinate and communicate with members of the interdisciplinary team in developing an individualized plan of care to meet identified spiritual needs and will provide direct spiritual care to patients and families/caregivers. Attends and actively participates in the interdisciplinary group meeting. Serves as liaison and support to community chaplains and spiritual counselors and documents ongoing communication. Provides for funeral or memorial services for patients as requested. Collaborates with the Bereavement Coordinator in planning periodic memorial services to meet the needs of personnel, volunteers, and community clergy/spiritual counselors working with the hospice team.

The Requirements:

  • Graduate of an accredited seminary or school of theology and/or certification, CPE preferred
  • Documented evidence of appropriate training and experience counseling and addressing grief issues of the bereaved under the supervision of a qualified professional.
  • Hospice/ experience preferred working with death and dying, individuals/family/caregiver.
