The Palliative Care Nurse Practitioner is a registered professional nurse with advanced graduate education and clinical training and who provides comprehensive care in compliance with the state Nurse Practice Act and under the supervision of a collaborating physician. At Palliative Care at Heart, the Nurse Practitioner will care for patients and their families living with advanced progressive or terminal illnesses, completing face-to-face assessments and evaluating palliative care eligibility. In collaboration with the Chief Medical Officer, the Nurse Practitioner will also develop practice protocols for end-of-life care, educate other clinicians about palliative care, conduct industry research, and fulfill any administrative responsibilities required at Palliative Care at Heart.

The Position:

The Palliative Care Nurse Practitioner will play an integral role in not only delivering expert care to patients who choose our services, but will also assist in attaining Palliative Care at Heart’s strategic clinical goals.

The Palliative Care Nurse Practitioner:

  • Offers medical expertise on pain, symptom management, and the evaluation of a terminal prognosis to patients, families, and staff
  • Orders therapies and tests as appropriate
  • Completes an average of 10-12 patient visits per day
  • Ensures care is provided based on established palliative care guidelines and standards
  • Collaborates with hospital/oncologist/pulmonologist providers regarding new patient referrals
  • Performs patient visits primarily in long-term care facilities, assisted living facilities, group homes, or homes
  • Works with attending physicians, as necessary, regarding the treatment of Palliative Care at Heart patients and regarding potential hospice referrals
  • Acts as the attending physician for identified hospice patients
  • Provides the Hospice Medical Director with face-to-face encounter documentation if visiting a hospice patient
  • Maintains and expands professional knowledge on an ongoing basis through seminars, conferences, and continuing education in-services

The Requirements:

  • Registered Nurse and Nurse Practitioner
  • Board Certification in Hospice and Palliative Care preferred
  • Current nursing licensure in Louisiana and in good standings with no restrictions
  • Accreditation from an approved certifying body for Advanced Practice Nursing as required by Louisiana
  • CPR certification
  • Minimum of one-year acute care experience, one-year Palliative Care experience, or three years of post-acute experience
  • Collaborative agreement with Hospice Medical Director filed with state licensing board
  • Ability to provide on-call coverage if necessary
  • Strong interpersonal and communication skills
