RN Case Managers

As a member of the Registered Nurse Case Manager Team, you will serve as a critical liaison between the benefit of hospice services and potential patients, their loved ones and caregivers, and our healthcare partners. Position:

The RN Case Manager plans, organizes, and directs hands-on, skilled nursing care by performing an initial comprehensive assessment and ongoing assessments of the needs of the patient and their family. A collaborative care plan is developed that establishes goals based on the interdisciplinary team’s recommendations and the patient’s desires.

Qualifications

Required Education, Experience, or Certification:

  • Must be a licensed Registered Nurse in good standings with no restrictions in the state
  • Must have Two (2) years’ experience as an RN in healthcare; with at least one of which is in public health, home care, or hospice is preferred.
  • Two years’ experience as a Registered Nurse
  • Current CPR certification
  • Home health or hospice experience (recommended)
