The Nurse Practitioner (PRN) provides comprehensive hospice services in collaboration with the attending physician or medical director to provide hospice care through the evaluation and assessment of the health history. The NP will make post-admission visits as well as help with face to face visits.

The Position:

The Nurse Practitioner is an integral part of the Hospice IDT and as such is responsible for the excellent delivery of care in every care setting after hours and during the weekend. This position is great for the individual that is motivated and self-directed.  If you are flexible and need flexibility in your current lifestyle then this is the job for you!!  

 The Requirements:

  • Graduate from an accredited School of Nursing and accredited Nurse Practitioner Program.
  • Current nursing licensure in the state in good standings with no restrictions.
  • Maintains current CPR certification.
  • Master's degree with a minimum of one (1) year Nurse Practitioner experience and a minimum of one (1) year hospice or palliative care experience.
  • Accreditation from an approved certifying body for advanced practice nursing as required by State.
  • Certification in a specialist area preferred; (e.g. Hospice and Palliative Nursing (CHPN), Pediatrics or Geriatrics).
  • Collaborative agreement with the hospice medical director completed and filed with the state licensing board.
