Position:

The RN Case Manager plans, organizes, and directs hands-on, skilled nursing care by performing an initial comprehensive assessment and ongoing assessments of the needs of the patient and their family. A collaborative care plan is developed that establishes goals based on the interdisciplinary team’s recommendations and the patient’s desires. We are looking for a compassionate, flexible, and organized Registered Nurse to work 7on/7off Days. To assist with overflow and late admissions to help the on-call nurse

 Qualifications:

  • Must be a licensed Registered Nurse in good standings with no restrictions in the state
  • Two years’ experience as a Registered Nurse
  • Current CPR certification
  • Home health or hospice experience (recommended)
