The Position:

Our Patient Care Managers lead a multi-disciplined team and care for 50-60 patients. The PCM is Responsible to plan, organize, and direct all patient care services, High level of visibility in the community and facilities his/her team services. Admissions Nurse conducts informational and needs assessments to determine eligibility and identify hospice needs. Communicates with the referring and/or attending physician regarding hospice eligibility and coordinates with Hospice Staff and Vendors to ensure patients’ needs. Develops a care plan that establishes goals as well as ordering of medications, DME and supplies related to the plan of care. Teaches the patient and family/caregiver self-care techniques as appropriate, Works with staff members of IDT to ensure collaboration of the patient’s Plan of Care.   Acts as a liaison with community referral sources, physicians, hospitals, and nursing homes to bring a greater understanding of the hospice philosophy and support to patients and families.

The Requirements:

  • Must be a licensed Registered Nurse in good standings with no restrictions in the state
  • Two (2) years’ experience in healthcare, with at least one of which is in the area of public health, home care, or hospice experience preferred
  • Current CPR certification
  • Self-directed and able to work with minimal supervision
  • Ability to address common concerns and overcome objectives
