Job Description Summary


The Crisis Care LPN is an integral part of the Hospice team and as such is responsible for the excellent delivery of care in every care setting after hours and during the weekend.


JOB Responsibilities


  • Receives calls from patients and/or family members and quickly respond to and access problems delivering high quality care in a crisis situation.
  • Provides direct patient nursing care following a comprehensive assessment and the plan of care under the supervision of the registered nurse.
  • Recognizes and interprets pain/symptom management and institute remedial measures within the scope of a Licensed Practical Nurse. Evaluates patient’s response to treatments/medications and reports significant changes in the patient condition to the registered nurse immediately and/or changes needed to the plan of care.
  • Assesses and provides patient and family/caregiver education and information pertinent to diagnosis and self-plan of care.
  • Participates in coordination of hospice services, appropriately reporting the identified needs to the interdisciplinary group.
  • Uses equipment and supplies efficiently and in a cost-effective manner to support the organization’s financial performance.
  • Assists the registered nurse in overseeingthe utilization and ordering of medications, DME and supplies related to the plan of care in a manner consistent with the Hospice’s philosophy of care. 
  • Participates in quality assessment performance improvement teams and activities.
  • Assists the patient with activities of daily living as needed.
  • Responsible to stay educated regarding all updates inEHR, regulations, documentation needs by attending mandatory in-services and educational trainings.
  • Responsible to communicate to registered nurse, medical director, attending physicians, etc regarding patient status changes or needs during afterhours call.
  • Acts as a liaison and educator where needed between patient/family, hospice team, andvendors (DME, Pharmacy, etc.).
  • Knowledge of and adherence to all policy and procedures.
  • Maintains compliance by documenting accurately and timely within the organization’s electronic health record.

  • Must be a Licensed Practical Nurse in good standings, in the state of Louisiana with no restrictions. 
  • Must have(2) years experience as a LPN. 
  • Community health/hospice or medical/surgical experience is preferred.
  • Knowledge of hospice philosophy, grief-loss, death and dying processes
  • Primarily frequent in nature with exerting up to fifty pounds of force occasionally, and/or up to twenty pounds of force frequently, and/or up to ten pounds of force constantly to move objects: reaching, lifting, carrying, handling, walking, sitting, standing, bending, squatting, climbing, kneeling, and twisting.  Position may require pushing and pulling up to 300 lbs. Frequent to continuous speaking, hearing, near visual acuity, far visual acuity, depth perception, typing and driving.
