On- Call RN 7/7

The On Call RN isan integral part of the Hospice IDT and as such isresponsible fortheexcellent delivery of care in every care setting after hours and during the weekend.   




  • Receives calls from patients and/or family members and quickly respond to and access problems delivering high quality care in a crisis situation. 
  • Completes an initial, comprehensive and ongoing assessments of patient and family to determine hospice needs including a complete physical assessment and history of current and previous illness (esand initiating a plan of care as needed. 
  • Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions including the patient and the family in the planning process. 
  • Assists in overseeingthe utilization and ordering of medications, DMEand supplies related to the plan of care in a manner consistent with Heart of Hospice’s philosophy of care. 
  • Initiates appropriate preventive and rehabilitative nursing procedures.  Administers medications and treatments as prescribed by the physician in the physician’s plan of care. 
  • Counsels the patient and family in meeting nursing and related needs. 
  • Provides health care instructions to the patient as appropriate per assessment and plan. 
  • Records pain/symptom management changes/outcomes as appropriate. 
  • Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required. 
  • Communicates with community health related persons to coordinate the care plan. 
  • Teaches the patient and family/caregiver self-care techniques as appropriate.  Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers.   
  • Uses equipment and supplies efficiently and in a cost effective manner to support the organizations financial performance. 
  • Assists the patient and family/caregiver and other team members in providing continuity of care. 
  • Attends and actively participates in weekly Interdisciplinary team meeting as assigned. 
  • Responsible to stay educated regarding all updates inEHR,regulations, documentation needs by attending mandatory in-services and educational trainings. 
  • Acts as a liaison and educator where needed between patient/family, hospice team, andvendors. 
  • Knowledge of and adherence to all policy and procedures. 
  • Maintains compliance by documenting accurately and timely within the organization’s electronic health record.
