We are currently looking for an On-Call Registered Nurse (RN) to join our team and carry out our vision of excellent care in the Fort Smith and River Valley area.

JOB OPENING

The On Call RN is an integral part of the Hospice IDT and as such is responsible for the excellent delivery of care in every care setting after hours and during the weekend.This position is great for the individual that is motivated and self-directed.  If you are flexible and need flexibility in your current lifestyle then this is the job for you!!  

Shift:  7 days on/7 days off

JOB REPSONSIBILITIES

  •  Receives calls from patients and/or family members and quickly respond to and access problems delivering high quality care in a crisis situation.
  • ·Completes an initial, comprehensive and ongoing assessments of patient and family to determine hospice needs including a complete physical assessment and history of current and previous illness (es) and initiating a plan of care as needed.
  •  Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions including the patient and the family in the planning process.
  •  Assists in overseeing the utilization and ordering of medications, DME and supplies related to the plan of care in a manner consistent with Heart of Hospice’s philosophy of care.
  •  Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care.
  •  Counsels the patient and family in meeting nursing and related needs.
  • Provides health care instructions to the patient as appropriate per assessment and plan.
  • Records pain/symptom management changes/outcomes as appropriate.
  •  Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required.
  •  Communicates with community health related persons to coordinate the care plan.
  •  Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers.
  •  Assists the patient and family/caregiver and other team members in providing continuity of care.

REQUIREMENTS

  • Must be a licensed Registered Nurse in good standings with no restrictions in the state
  • Two (2) years experience as a Registered Nurse. Hospice experience a plus
  • Current CPR certification
