Job Description Summary

 

This role will be a Full Time dual role of Social worker and Volunteer Coordinator. The Social Worker is responsible for ensuring the social service needs of the patients and caregivers are met. The volunteer coordinator is responsible for planning, coordinating, and managing all aspects of the organizations Volunteer Program.

 

 

JOB Responsibilities

 

Social Worker

  • Assessing the psychosocial status of patients and families/caregivers related to the patient’s illness and environment and communicated finds to the interdisciplinary group.
  • Providing safety assessment in the patient’s home setting and plans interventions based on evaluation findings.
  • Provides information and serves as liaison between patients and families/caregivers and community agencies
  • Maintains and develops relationship with organization personnel to support patient care
  • Participates in discharge planning and in-serve programs related to the needs of the patient
  • Provides information and referral services for organization patients and families/caregivers regarding practical and environmental needs.

 

 

Volunteer Coordinator

  • Develop and maintain volunteer training and orientation program
  • Recruits, selects, and orients volunteers ensuring that the volunteer understands they are responsible to meet the needs of the patient.
  • Establish a need for volunteer services with the patient and family/caregiver and entire hospice team
  • Responsible for accessing and documenting the competency of volunteers at the time of interview, at the completion of the initial orientation and annually.
  • Maintains monthly statistics including the cost savings achieved through the use of volunteers and the level of activity.
  • Maintains accurate and current volunteer personnel files.
  • Effectively documents recruitment efforts on an ongoing basis

 

 

JOB REQUIREMENTS

  • A master’s degree from a school of social work accredited by the Council on Social Work Education.  LCSW preferred.
  • Minimum of one year's experience in health care
  • Documented clinical experience appropriate to the counseling and casework needs of the terminally ill; Hospice experience preferred.
  • Hospice and/or volunteer experience preferred
