We are currently looking for a Full-Time Registered Nurse (RN) Case Manager to join our team and carry out our vision of excellent care in the Fort Smith and River Valley area.

 

POSITION DETAILS

The Registered Nurse (RN) Case Manager plans, organizes, and directs hands-on skilled nursing care by evaluating and performing initial comprehensive and ongoing assessments of the patient and family to determine hospice needs. A collaborative care plan is developed that establishes goals, based on the interdisciplinary team’s recommendations and the patient’s wants.

REQUIREMENTS

  • Non-restricted Arkansas RN license
  • Current CPR certification
  • Two years’ experience as a Registered Nurse
  • Home health or hospice experience, a plus
