We are currently looking for an On-Call Registered Nurse (RN) to join our team and carry out our vision of excellent care in the Fort Smith and River Valley area.

JOB DESCRIPTION SUMMARY

The Admissions Nurse is an integral part of the Hospice Interdisciplinary Team and is responsible for the needs assessment and admission of eligible patients that desire our care.

Shift: 7 days on / 7 days off

 

JOB RESPONSIBILITIES

 

  • Conducts informational and needs assessments in addition to comprehensive and ongoing assessments of patient, family, and other healthcare providers to determine eligibility and identify hospice needs.
  • Coordinates with Hospice Staff and Vendors to ensure patients’ needs are addressed and met during the admission process and needs assessment process.
  • Provides education and health care instructions to the patient and family as appropriate in meeting nursing and related needs per assessment and plan of care.
  • Maintains and records pain/symptom management changes/outcomes as appropriate.
  • Assists in overseeing the utilization and ordering of medications, DME and supplies related to the plan of care in a manner consistent with the needs of the patient and the organizations performance goals.
  • Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers.
  • Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions respecting the patient and family wishes by including them in the planning process.
  • Provides excellent and professional nursing care by utilizing all elements of nursing process.
  • Communicates with community health related persons to coordinate the care plan as needed.
  • Communicates with the referring and/or attending physician regarding hospice eligibility and patient’s needs to establish the Plan of Care. Obtains/receives physicians’ orders as required.
  • Works with all Hospice Staff and members of IDT to ensure collaboration of the patient’s Plan of Care.
  • Acts as a liaison with community referral sources, physicians, hospitals, and nursing homes to bring a greater understanding of the hospice philosophy and support to patients and families.
  • Actively participates in quality assessment performance improvement teams and activities.
  • Knowledge of and adherence to all policy and procedures.
  • Maintains compliance by documenting accurately and timely within the organization’s electronic health record.

JOB REQUIREMENTS

  • Registered nurse with current licensure with no restrictions to practice professional nursing in the state.
  • Maintains a current CPR certification.
  • Minimum of two years’ experience in healthcare, at least one of which is in the area of public health, home care, or hospice nursing is preferred.
  • Self-directed and able to work with minimal supervision.
  • Ability to address common concerns and overcome objectives.
  • Primarily frequent in nature with exerting up to fifty pounds of force occasionally, and/or up to twenty pounds of force frequently, and/or up to ten pounds of force constantly to move objects: reaching, lifting, carrying, handling, walking, sitting, standing, bending, squatting, climbing, kneeling, and twisting.  Position may require pushing and pulling up to 300 lbs. Frequent to continuous speaking, hearing, near visual acuity, far visual acuity, depth perception, typing and driving.
