Job Description Summary

PRN position with potential to become full time. The Registered Nurse Case Manager plans, organizes and directs hospice care and is experienced in nursing, with emphasis on community health education/experience.  The registered nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families in their home setting.

Position:

The RN Case Manager plans, organizes, and directs hands-on, skilled nursing care by performing an initial comprehensive assessment and ongoing assessments of the needs of the patient and their family. A collaborative care plan is developed that establishes goals based on the interdisciplinary team’s recommendations and the patient’s desires.

 

Qualifications

Required Education, Experience, or Certification:

  • Non-restricted Louisiana RN license
  • Two years’ experience as a Registered Nurse
  • Current CPR certification
  • Home health or hospice experience (recommended)
