Job Opening

We are looking for an experienced, compassionate, caring, and dynamic Full-Time Social Worker. Our Social Workers are responsible for the provision of psychosocial care services to patients and families/caregivers  

Duties and Responsibilities:

  • Assesses the psychosocial status of patients and families/caregivers related to the patient's terminal illness and environment and communicates findings to the interdisciplinary group in establishing the plan of care.
  • Provides safety assessment in the patient's home setting and plans interventions based on evaluation findings.
  • Provides education and training to the patient and primary care giver as needed in relationship to stress and other identified coping difficulties provided in the plan of care.
  • Provides information and referral services for organization patients and families/caregivers regarding practical and environmental needs.
  • Provides information and serves as liaison between patients and families/caregivers and community agencies.
  • Maintains collaborative relationships with organization personnel to support patient care.
  • Maintains and develops contracts with public and private agencies as resources for patient and personnel.
  • Participates in the development of the individualized plan of care and attends and participates in regularly scheduled interdisciplinary group meetings.
  • Assists physician and other team members in understanding significant social and emotional factors related to health problems and death/dying issues.
  • Actively participates in quality assessment performance improvement teams and activities.
  • Participates in discharge planning and in-service programs related to the needs of the patient.


* A master’s degree from a school of social work accredited by the Council on Social Work Education. LCSW preferred.

* Minimum of one year's experience in health care, hospice experience preferred.

* Documented clinical experience appropriate to the counseling and casework needs of the terminally ill; Hospice experience preferred.
