OB DESCRIPTION SUMMARY 

 

The Chaplain is responsible for the provision of spiritual care services to patients and families/caregivers of the hospice, either directly or through coordination of care with other spiritual counselors. 

 

JOB RESPONSIBILITIES 

 

  • Completes a spiritual assessment of patients and families/caregivers in the hospice program meeting them where they are on their journey. 
  • Coordinates and communicates with members of the interdisciplinary team in developing an individualized plan of care to meet identified spiritual needs. 
  • Documents spiritual services provided and progress of patient/family needs on a clinical progress note. 
  • Provides direct spiritual care to patients and families/caregivers according to the plan of care. 
  • Maintains records of spiritual care services utilization and related activities for quality assurance and program development. 
  • Attends and actively participates in the interdisciplinary group meeting. 
  • Assists in supervision of spiritual care volunteers when assigned to patients/ families/caregivers. 
  • Serves as liaison and support to community chaplains and spiritual counselors and documents ongoing communication. 
  • Recruits community chaplains, spiritual counselors, and spiritual care volunteers adequate to meet patient and family/caregiver needs by developing community contacts and offering education through congregations about hospice care. 
  • Actively participates in quality assessment performance improvement teams and activities. 
  • Provides consultation, education, and support to the interdisciplinary group and hospice staff on spiritual care. 
  • Provides for funeral or memorial services for patients as requested. 
  • Collaborates with the Bereavement Coordinator in planning periodic memorial services to meet the needs of personnel, volunteers, and community clergy/spiritual counselors working with the hospice team. 
  • Knowledge of and adherence to all policy and procedures. 
  • Maintains compliance by documenting accurately and timely within the organization’s electronic health record.

JOB REQUIREMENTS 

  • Graduate of an accredited seminary or school of theology and/or certification, CPE preferred. 
  • Documented evidence of appropriate training and skills to provide spiritual counseling, such as Bachelor of Divinity, Master of Divinity or equivalent theological degree or training. 
  • Experience working with death and dying, individuals/family/caregiver preferred. 
  • Hospice/ experience preferred. 
  • Primarily frequent in nature with exerting up to fifty pounds of force occasionally, and/or up to twenty pounds of force frequently, and/or up to ten pounds of force constantly to move objects: reaching, lifting, carrying, handling, walking, sitting, standing, bending, squatting, climbing, kneeling, and twisting.  Position may require pushing and pulling up to 300 lbs. Frequent to continuous speaking, hearing, near visual acuity, far visual acuity, depth perception, typing and driving.
